Making a referral to stop smoking services

Some Trusts or organisations have their own established processes for referring people for stop smoking support. Please use those if your organisation has them. Otherwise, please see the information below
Derby residents or those who have a Derby GP
Staff with an nhs.net or derby.gov.uk account can refer to the Livewell service by email.
Please send the person’s full name, date of birth, address, GP surgery, and telephone number to livewell@derby.gov.uk
Professionals without an nhs.net or derby.gov.uk account - please signpost to the Livewell stop smoking service.
Derbyshire residents or those who have a Derbyshire GP
All professionals can complete the secure form to refer to the Live Life Better Derbyshire service.
